MedjetAssist’s Membership Eligibility

  • Must be under age 75 for basic membership.
  • A family membership shall include the named primary member, his or her spouse and up to five of their unmarried dependent children (including step, foster or legally adopted children) under the age of 19 or up to age 23, if a full-time student in actual attendance at an accredited school or college and dependent on the primary member for support and maintenance. If a birth or legal adoption occurs during the year of a family membership, the new dependent is automatically covered upon notification. A dependent child, as used above, is one who lives regularly and permanently with the primary member in a parent-child relationship and is chiefly dependent upon the member for support.
  • Membership enrollment and renewal are subject to approval by the MedjetAssist membership committee.
  • Membership is valid only if the appropriate membership fee is collected.
  • Members who have reached age 75 or older at the time of their enrollment are not eligible for membership benefits under the Basic Plan. They can get the MedjetAssist Diamond Plan. All applications must be reviewed and approved prior to enrollment for individuals age 75 – 85. All membership applications and / or enrollment forms must include accurate dates of birth in order to ensure eligibility for benefits. Note: The Diamond Plan may not be purchased online. Here’s Some More Important Information About the Diamond Plan:

  • A general health questionnaire and medical statement must be completed by the prospective member and his or her physician. Only the Medical Statement form provided by MedjetAssist will be accepted for consideration. All information is kept strictly confidential.
  • Diamond Plan membership plans are available only as individual memberships at a cost of $395.00 per person, per year. If a prospective Diamond Plan member is currently enrolled in a basic family membership, and the spouse has not yet reached age 75, we will enroll the spouse (and dependents, if applicable) on a separate membership at their regular rate.

Diamond Plan members are entitled to the same MedjetAssist benefits as basic members with the following exceptions:

  • Diamond Plan members are limited to one transport per membership year.
  • Member transport benefits for Diamond Plan members are subject to the following preexisting condition limitation:“Membership benefits will not be provided for any preexisting defect, infirmity or condition for which an enrolled member is receiving medical treatment, advice or consultation in which there has been any (1) alteration in the dosage or usage of prescribed medication for such condition or (2) alteration or modification to an existing treatment plan for such condition or (3) invasive or noninvasive treatment or therapy for such condition, either as an inpatient or outpatient during the 180-day (6-month) period immediately prior to the effective date of the membership. Benefits will be provided if the member’s preexisting defect, infirmity or condition has been controlled without changes as listed above for longer than period shown, and such sickness or injury is unexpected.”

MedjetAssist’s Air Medical Transport Criteria

  • You must be hospitalized as an inpatient more than 150 miles from your primary residence as listed on your enrollment application and continue to meet inpatient criteria at the receiving hospital.
  • If you are medically discharged from the hospital or leave against medical advice and are physically able to travel under you own power, you will no longer be eligible for air medical transport benefits.
  • Due to the limited medical facilities and testing available on cruise ships, in some cases the MedjetAssist Medical Director may require the member to be admitted to a hospital on-shore before scheduling air medical transportation to another hospital.
  • All arrangements for medical evacuation and repatriation must be made by MedjetAssist. Because MedjetAssist is an assistance program and not an insurance plan, we will not reimburse members for expenses they incur on their own. Decisions regarding urgency of the case, the best timing and the most suitable means of transportation will be made by the MedjetAssist medical department after consulting with the local attending physician and the receiving physician.
  • Evacuation flights will be performed if an inpatient hospitalization is required, and the remaining in-patient hospital stay can be completed at a hospital of the member’s choice near the member’s home, and the member is unable to return to his home hospital via commercial airline without medical escort. Aircraft used for the medical transport of MedjetAssist members are fully equipped intensive care aircraft staffed with specially trained medical teams. However, if the member’s condition permits, the member may be transported by scheduled commercial airline, while in the care of a MedjetAssist medical team. Air medical services are limited to two separate flights per membership per year, except for repatriation flights involving enrolled, multiple family members requiring simultaneous repatriation. Under these circumstances, each family member will receive one fully paid flight.
  • Transport from home hospital to a specialty hospital more than 150 miles away is provided only in cases in which the appropriate medical specialty or equipment is not available at the member’s hometown hospital and the member cannot travel commercially without medical escort. (This benefit does not apply to short-term membership.)

MedjetAssist members are transported without regard to medical necessity. Members, as long as they are hospitalized as an inpatient more than 150 miles from their home, can choose to be transported to another hospital, even if the facility they are in is considered adequate. The member makes the decision to be transported.

Many health, travel insurance plans and platinum card benefits require that a transfer be medically required. In other words, as long as the care at a hospital is deemed adequate, transferring to another hospital is not an option.

You Can Go To The Hospital Of Your Choice
MedjetAssist flies individuals to their home hospital or hospital of their choice. Many plans generally restrict transports to the nearest hospital that provides “appropriate care.” This means individuals are taken to a hospital the shortest distance away that has adequate care, but not necessarily the best care.

MedjetAssist members, if hospitalized, do not have to worry about being transported to a nearby hospital in a foreign country that has only adequate medical care. Members choose if they want to fly home to their local hospital or hospital of their choice to receive the very best care and to be near their family.

There Are No Cost Limits
MedjetAssist has no limit on the cost of a medical transport. Many companies and insurance plans place caps – some as low as $25,000 – on the total amount available for an air ambulance. But a medical transport between Europe and America can run more than $35,000. Middle East and South American flights range from $60,000 to $80,000. Transport from Asia often exceeds $100,000.

You Have Domestic & Foreign Transport Coverage
MedjetAssist provides air medical transportation within the United States as well as abroad. If a MedjetAssist member becomes ill or injured and is hospitalized while traveling in the U.S. or abroad, MedjetAssist will take the member to their home hospital or the hospital of their choice.

In addition, if a MedjetAssist member is hospitalized in their hometown hospital and requires the services that only a specialty hospital (center of excellence) can provide, and the member is too ill or injured to fly commercially, MedjetAssist will fly that member to the specialty hospital, provided it is more than 150 miles from the member’s home hospital.

MedjetAssist Air Transport Restrictions and Exclusions

Yes, MedjetAssist does have Air Transport Exclusions & Restrictions. It’s a good idea to see what’s not covered

**EXCLUSIONS**:

Members will not be entitled to air medical transport benefits if their illnesses or injuries are the results of or are contributed to by the following:

  • War, invasion or civil war;
  • Suicide or attempted suicide or intentional self injury;
  • A member’s own criminal or felonious act or sustained while the member is in a state of insanity;
  • A member on an organ transplant list prior to enrollment will not be entitled to a transport for that transplant;
  • A member who is hospitalized at the time of enrollment will not be eligible for transport benefits for that hospitalization and may not be accepted for membership entirely;

MedjetAssist reserves the right to change or amend these rules and regulations. MedjetAssist is solely responsible for the interpretation and application of the rules and regulations communicated in this publication. All determinations by MedjetAssist shall be final and conclusive in each case. Memberships are non-transferable and non-refundable. By enrolling as a member, you accept and agree to the terms and conditions of membership.

**RESTRICTIONS**:

  • Both the originating and receiving hospitals must be reasonably accessible by ground ambulance to transport the member to and from an airfield capable of accommodating an authorized aircraft.
  • Membership provides access to MedjetAssist authorized affiliates only.
  • Due to the high risk of sending U.S. registered aircraft and personnel into countries where the U.S. State Department has issued travel restrictions, membership services are subject to exclusion in these areas.
  • A member with tuberculosis or other chronic airborne pathogens may not be transported.
  • A member with mild lesions, simple injuries such as sprains, simple fractures or mild illness which can be treated by local doctors and do not prevent the member from continuing his or her trip or returning home does not qualify for air medical transport.
  • Air medical transport services will not be provided in cases where the member’s primary admitting diagnosis is an inpatient psychiatric disorder.
  • A member traveling outside the United States for the sole purpose of seeking medical treatment, whether inpatient or outpatient, experimental or otherwise, will not be eligible for air medical transport benefits for that specific trip.
  • A member traveling or living outside the United States or Canada for an uninterrupted period in excess of 90 days is not eligible for membership benefits under the standard plan. Extended Stay (Expatriate) packages are available upon request.
  • Up to two (2) family members, business associates and/or traveling companions may accompany the patient, at no additional cost, on a MedjetAssist authorized aircraft during a transport, provided space is available and the patient care is not compromised. One family member or traveling companion may accompany patients being transported via scheduled commercial airline at no additional cost via economy class.
  • Payment of membership fee entitles the member to the membership services as published at no additional cost to the member. This membership permits MedjetAssist to collect for services rendered directly from the member’s insurer or other provider, if available. If the members has no insurance coverage for the services rendered, the member is still entitled to all MedjetAssist benefits at no additional cost.
  • While MedjetAssist makes every effort to accommodate its members, the patient and an accompanying passenger are limited to one small carry-on bag each due to limited space available on medical aircraft. MedjetAssist will arrange for additional luggage to be forwarded at the member’s expense.

My name is Mickie. I’d like to share our experience with Snowbirdinsurance.com and Steve Dasseos.

For the past 10+ years, my Grandparents have been escaping the Alberta winters by “Snowbirding” to Arizona for 5 months.

My Grandfather had some health issues requiring hospitalization (in Alberta) during the summer of 2010. He recovered and was looking forward to their planned travel back to Arizona in Oct of 2010. After giving his business to the same insurance provider for the past 10+ years, he was flat out refused for ANY insurance coverage by them, due to his hospitalization that summer. At the age of 88, we found that most insurance providers refused him insurance or the prices were extremely high with very little coverage and many confusing “sub-clauses”.

After scouring the internet, we came across SnowbirdInsurance.com. Admittedly, at first, the My Travel Guard policy seemed too good to be true.

The ability to waive the 180 day look back period, regardless of his medical issue over the summer, seemed too good to be true. After asking many questions to a very patient Steve Dasseos, we were satisfied that we understood the policy and he purchased the “My Travel Guard” plan for $100,000 coverage.

In March of 2011, just days before my Grandparents were flying home to Alberta, my Grandfather was rushed to hospital in Arizona needing emergency surgery.

The insurance would be put to the test.

All of the hospital, doctor and surgery expenses were fully covered and it was without question or hassle. Travel Guard had a team of people that liaised between our family and the hospital to ensure my Grandfather received all he needed to recover. As a family, we felt huge relief that he was going to be okay, and after surgery and a 5 day stay in the hospital, was released and able to return back home to Alberta the first week of April without any medical expenses to pay.

Fast forward to Aug 2011. My Grandparents hoped to travel back to Arizona again for the 2011/2012 winter. I expected there to be some issue or limitation with his coverage since he now had a claim history. We were pleasantly surprised to find that his insurance cost did not increase at all and his coverage is as comprehensive as the previous season. My Grandmother chose to use SnowbirdInsurance.com as well and their flights are booked!

Does OHIP Cover You Outside Canada?

When I travel outside of Canada will OHIP pay the same medical expenses that are covered in Ontario?

No. If you are a resident of Ontario and you are insured under OHIP, you are entitled to very limited funding for a limited range of medical services when you are travelling outside of Canada. For this reason, you are strongly advised to purchase additional health insurance every time you leave Canada.

OHIP will not pay more than the service would have cost in Ontario.

  • For physician services, OHIP will pay the actual cost billed by the out-of-country physician(s) or the cost of the same physician service(s) in Ontario, whichever is less. Physician services in Ontario are usually rendered at a significantly lower cost than at those billed out-of-country health facilities. Please note that out-of-country health facilities and physicians usually bill on an individual basis.
  • For health facility services, OHIP will pay $50 Canadian (CDN) per day.
  • For inpatient services, OHIP will pay $200 CDN per day. If the services are inpatient services rendered in an operating room, coronary care unit, intensive care unit, neonatal or pediatric special care unit, then OHIP will pay at the higher rate of $400 CDN per day for hospital services.

    Who Is A Family Member (for the Family coverages)?

    A family is a primary member, spouse or partner residing at the same location and up to 5 unmarried dependent children under the age of 19 unless they are a full-time student then up to age 23.

    MedjetAssist’s Membership Eligibility

    • Must be under age 75 for basic membership.
    • A family membership shall include the named primary member, his or her spouse and up to five of their unmarried dependent children (including step, foster or legally adopted children) under the age of 19 or up to age 23, if a full-time student in actual attendance at an accredited school or college and dependent on the primary member for support and maintenance. If a birth or legal adoption occurs during the year of a family membership, the new dependent is automatically covered upon notification. A dependent child, as used above, is one who lives regularly and permanently with the primary member in a parent-child relationship and is chiefly dependent upon the member for support.
    • Membership enrollment and renewal are subject to approval by the MedjetAssist membership committee.
    • Membership is valid only if the appropriate membership fee is collected.

    MedjetAssist’s Air Medical Transport Criteria

    • You must be hospitalized as an inpatient more than 150 miles from your primary residence as listed on your enrollment application and continue to meet inpatient criteria at the receiving hospital.
    • If you are medically discharged from the hospital or leave against medical advice and are physically able to travel under you own power, you will no longer be eligible for air medical transport benefits.
    • Due to the limited medical facilities and testing available on cruise ships, in some cases the MedjetAssist Medical Director may require the member to be admitted to a hospital on-shore before scheduling air medical transportation to another hospital.
    • All arrangements for medical evacuation and repatriation must be made by MedjetAssist. Because MedjetAssist is an assistance program and not an insurance plan, we will not reimburse members for expenses they incur on their own. Decisions regarding urgency of the case, the best timing and the most suitable means of transportation will be made by the MedjetAssist medical department after consulting with the local attending physician and the receiving physician.
    • Evacuation flights will be performed if an inpatient hospitalization is required, and the remaining in-patient hospital stay can be completed at a hospital of the member’s choice near the member’s home, and the member is unable to return to his home hospital via commercial airline without medical escort. Aircraft used for the medical transport of MedjetAssist members are fully equipped intensive care aircraft staffed with specially trained medical teams. However, if the member’s condition permits, the member may be transported by scheduled commercial airline, while in the care of a MedjetAssist medical team. Air medical services are limited to two separate flights per membership per year, except for repatriation flights involving enrolled, multiple family members requiring simultaneous repatriation. Under these circumstances, each family member will receive one fully paid flight.
    • Transport from home hospital to a specialty hospital more than 150 miles away is provided only in cases in which the appropriate medical specialty or equipment is not available at the member’s hometown hospital and the member cannot travel commercially without medical escort. (This benefit does not apply to short-term membership.)

    More Information:

    No Medical Necessity To Be Transported
    MedjetAssist members are transported without regard to medical necessity. Members, as long as they are hospitalized as an inpatient more than 150 miles from their home, can choose to be transported to another hospital, even if the facility they are in is considered adequate. The member makes the decision to be transported.

    Many health, travel insurance plans and platinum card benefits require that a transfer be medically required. In other words, as long as the care at a hospital is deemed adequate, transferring to another hospital is not an option.

    You Can Go To The Hospital Of Your Choice
    MedjetAssist flies individuals to their home hospital or hospital of their choice. Many plans generally restrict transports to the nearest hospital that provides “appropriate care.” This means individuals are taken to a hospital the shortest distance away that has adequate care, but not necessarily the best care.

    MedjetAssist members, if hospitalized, do not have to worry about being transported to a nearby hospital in a foreign country that has only adequate medical care. Members choose if they want to fly home to their local hospital or hospital of their choice to receive the very best care and to be near their family.

    There Are No Cost Limits
    MedjetAssist has no limit on the cost of a medical transport. Many companies and insurance plans place caps – some as low as $25,000 – on the total amount available for an air ambulance. But a medical transport between Europe and America can run more than $35,000. Middle East and South American flights range from $60,000 to $80,000. Transport from Asia often exceeds $100,000.

    You Have Domestic & Foreign Transport Coverage
    MedjetAssist provides air medical transportation within the United States as well as abroad. If a MedjetAssist member becomes ill or injured and is hospitalized while traveling in the U.S. or abroad, MedjetAssist will take the member to their home hospital or the hospital of their choice.

    In addition, if a MedjetAssist member is hospitalized in their hometown hospital and requires the services that only a specialty hospital (center of excellence) can provide, and the member is too ill or injured to fly commercially, MedjetAssist will fly that member to the specialty hospital, provided it is more than 150 miles from the member’s home hospital.

    MedjetAssist Air Transport Restrictions and Exclusions

    Yes, MedjetAssist does have Air Transport Exclusions & Restrictions. It’s a good idea to see what’s not covered

    **EXCLUSIONS**:

    Members will not be entitled to air medical transport benefits if their illnesses or injuries are the results of or are contributed to by the following:

    • War, invasion or civil war;
    • Suicide or attempted suicide or intentional self injury;
    • A member’s own criminal or felonious act or sustained while the member is in a state of insanity;
    • A member on an organ transplant list prior to enrollment will not be entitled to a transport for that transplant;
    • A member who is hospitalized at the time of enrollment will not be eligible for transport benefits for that hospitalization and may not be accepted for membership entirely;

    MedjetAssist reserves the right to change or amend these rules and regulations. MedjetAssist is solely responsible for the interpretation and application of the rules and regulations communicated in this publication. All determinations by MedjetAssist shall be final and conclusive in each case. Memberships are non-transferable and non-refundable. By enrolling as a member, you accept and agree to the terms and conditions of membership.

    **RESTRICTIONS**:

    • Both the originating and receiving hospitals must be reasonably accessible by ground ambulance to transport the member to and from an airfield capable of accommodating an authorized aircraft.
    • Membership provides access to MedjetAssist authorized affiliates only.
    • Due to the high risk of sending U.S. registered aircraft and personnel into countries where the U.S. State Department has issued travel restrictions, membership services are subject to exclusion in these areas.
    • A member with tuberculosis or other chronic airborne pathogens may not be transported.
    • A member with mild lesions, simple injuries such as sprains, simple fractures or mild illness which can be treated by local doctors and do not prevent the member from continuing his or her trip or returning home does not qualify for air medical transport.
    • Air medical transport services will not be provided in cases where the member’s primary admitting diagnosis is an inpatient psychiatric disorder.
    • A member traveling outside the United States for the sole purpose of seeking medical treatment, whether inpatient or outpatient, experimental or otherwise, will not be eligible for air medical transport benefits for that specific trip.
    • A member traveling or living outside the United States or Canada for an uninterrupted period in excess of 90 days is not eligible for membership benefits under the standard plan. Extended Stay (Expatriate) packages are available upon request.
    • Up to two (2) family members, business associates and/or traveling companions may accompany the patient, at no additional cost, on a MedjetAssist authorized aircraft during a transport, provided space is available and the patient care is not compromised. One family member or traveling companion may accompany patients being transported via scheduled commercial airline at no additional cost via economy class.
    • Payment of membership fee entitles the member to the membership services as published at no additional cost to the member. This membership permits MedjetAssist to collect for services rendered directly from the member’s insurer or other provider, if available. If the members has no insurance coverage for the services rendered, the member is still entitled to all MedjetAssist benefits at no additional cost.
    • While MedjetAssist makes every effort to accommodate its members, the patient and an accompanying passenger are limited to one small carry-on bag each due to limited space available on medical aircraft. MedjetAssist will arrange for additional luggage to be forwarded at the member’s expense.

    Acute Onset of a Pre-Existing Condition: Acute Onset of a Pre-Existing Condition: If you are under age 70, you are covered for an Acute Onset of a Pre-existing Condition. Coverage is available up to $15,000 Maximum for Eligible Medical Expenses and up to $25,000 for Emergency Medical Evacuation. An Acute Onset of a Pre-existing Condition is a sudden and unexpected outbreak or recurrence of a Pre-existing Condition which occurs spontaneously and without advance warning either in the form of Physician recommendations or symptoms.     What is a Pre-Existing Condition? – A Pre-existing Condition is any Illness, Injury or medical condition or chronic or recurring Illness or Injury or medical condition, including any associated complications or consequences, which existed at or during the 2 years immediately preceding your Effective Date. An Acute Onset is a sudden and unexpected outbreak or recurrence of a Pre-existing Condition, that occurs spontaneously and without advance warning either in the form of Physician recommendations or symptoms. Treatment must be obtained within 24 hours of the sudden and unexpected outbreak or recurrence.

     

    Eligibility

    Atlas America – Only non-US Citizens who are at least 14 days of age are eligible for coverage under this plan. Individuals age 70 to 79 as of the Certificate Effective Date are subject to a $50,000 Overall Maximum. Individuals age 80 and over as of the Certificate Effective Date are subject to a $10,000 Overall Maximum.

    Atlas International – Only US Citizens and non-US citizens whose travel does not include the US, except US citizens whose travel includes an Incidental Trip to the US, and who are at least 14 days of age are eligible for coverage under this plan. Individuals age 70 to 79 as of the Certificate Effective Date are subject to a $50,000 Overall Maximum. Individuals age 80 and over as of the Certificate Effective Date are subject to a $10,000 Overall Maximum.

    Home Country Coverage

    Benefit Period – In the event a Member begins a Benefit Period while the Certificate is in effect, and the Certificate terminates because the Member returns to his/her Home Country, Underwriters will pay Eligible Medical Expenses, as defined herein, which are incurred in the Member’s Home Country during the Benefit Period. Home Country Coverage applies only to Eligible Medical Expenses.

    End of Trip Home Country Coverage – In the event a Member is covered hereunder and is outside his or her Home Country continuously for six (6) months or more, the Member may purchase an additional 30 days of End of Trip Home Country Coverage. Home Country Coverage applies only to Eligible Medical Expenses.

    Incidental Home Country Coverage – For each three (3) months during which a Member is covered hereunder, Medical Expenses only are covered during incidental trips totaling no more than 15 days duration per three-month period of coverage. Incidental visit time must be used within the three-month period earned, and the Member must continue his or her international trip in order to be eligible for this benefit.     When Does Coverage Become Effective and When Does it End?
    Your coverage becomes effective on the latest of: the moment we receive your Application and correct premium (if Application and payment is made online or by fax), 12:01am* on the date we receive your Application and payment (if Application and payment is made by mail), the moment you depart from your Home Country, or 12:01am on the date you request on your Application. Your coverage will end on the earliest of: 12:01am on the last day of the period for which you have paid a premium, 12:01am on the date requested on your Application, or the moment of your arrival upon return to your Home Country (unless you have started a Benefit Period or are eligible for Home Country Coverage).

    Does the Atlas Series Provide any Home Country Coverage?
    Yes. Under certain circumstances, the Atlas Series will provide limited Home Country Coverage.

    Incidental Home Country Coverage – The Atlas Series will provide you 15 days of incidental coverage for trips to your Home Country for every 3 months of coverage purchased. Incidental visit time must be used within the three-month period earned, and you must continue your international trip in order to be eligible for this benefit, which covers Medical expenses only.Return to your Home Country must not be taken for the purpose of obtaining treatment of an Illness or Injury that began while traveling.

    Benefit Period Medical Coverage – A Benefit Period begins on the first date you receive a diagnosis or treatment of a covered Illness or Injury while outside your Home Country and lasts for 180 days. If you started a Benefit Period while this insurance was in effect, you are covered only for Medical expenses for the duration of the Benefit Period, regardless of whether you are at home or abroad.

    End of Trip Home Country Medical Coverage – If you are covered under the Atlas Series and outside of your Home Country continuously (except for covered Incidental Trips as described above) for six (6) months or more you may purchase an additional 30 days of End of Trip Home Country Medical Coverage.

    Home Country Defined – If you are a US citizen, your Home Country is the United States, regardless of the location of your Principal Residence. If you are not a US citizen, your Home Country is the country where you principally reside and receive regular mail.

    Which Plan Should I Purchase?
    US citizens, as well as non-US citizens traveling outside of the US, should purchase Atlas International, which provides coverage outside of the US. Non-US citizens traveling to the United States should purchase Atlas America.
    What is Covered?
    All benefits, except Hospital Indemnity, Lost Checked Luggage, Natural Disaster, Accidental Death & Dismemberment and Common Carrier Accidental Death, are subject to the Deductible and Coinsurance. Limits apply to all benefits (See Schedule of Benefits and Limits):Medical:

    1. Inpatient and Outpatient charges made by a Hospital
    2. Charges made by a Physician, surgeon, radiologist, anesthesiologist, and any other medical specialist to whom the Physician has referred the case
    3. Charges made for dressings, sutures, casts or other supplies prescribed by the attending Physician or specialist, but excluding nebulizers, oxygen tanks, diabetic supplies and all devices for repeat use at home
    4. Charges for diagnostic testing using radiology, ultrasonographic or laboratory services
    5. Charges for oxygen and other gases and anesthetics and their administration
    6. Charges for prescription drugs for treatment of a covered Injury or Illness, but not for the replacement of lost, stolen, damaged, expired or otherwise compromised drugs
    7. Charges made by a licensed Extended Care Facility upon direct transfer from an acute care Hospital
    8. Emergency local ambulance transport incurred in connection with Injury or Illness resulting in inpatient hospitalization

    Complications of Pregnancy:

    Treatment of Complications of Pregnancy during the first 26 weeks of Pregnancy is covered under this insurance. Complications of Pregnancy is defined as: Illnesses whose diagnoses are distinct from Pregnancy, but are adversely affected by Pregnancy or caused by Pregnancy, and not associated with a normal Pregnancy. This includes: ectopic Pregnancy, spontaneous abortion, hyperemesis gravidarum, pre-eclampsia, eclampsia, missed abortion and conditions of comparable severity.

    Hospital Indemnity:

    If you are hospitalized as an Inpatient for treatment of a covered Illness or Injury, the Atlas Series will provide $100 for each night you spend in the hospital. This benefit is in addition to payments for other covered expenses and is not subject to Deductible or Coinsurance.

    Acute Onset of a Pre-Existing Condition:

    If you are under age 70, you are covered for an Acute Onset of a Pre-existing Condition. Coverage is available up to $15,000 Maximum for Eligible Medical Expenses and up to $25,000 for Emergency Medical Evacuation. An Acute Onset of a Pre-existing Condition is a sudden and unexpected outbreak or recurrence of a Pre-existing Condition which occurs spontaneously and without advance warning either in the form of Physician recommendations or symptoms. Treatment must be obtained within 24 hours of the sudden and unexpected outbreak or recurrence.

    Emergency Dental:

    The following Emergency Dental expenses are covered: Emergency Dental treatment and Dental surgery necessary to restore or replace sound natural teeth lost or damaged in an Accident that is covered under this insurance subject to the Overall Maximum Limit; and Emergency Dental Treatment necessary to resolve acute, spontaneous and unexpected onset of pain subject to a maximum benefit of $100.

    Political Evacuation:

    If the United States government issues a travel warning that becomes effective after your arrival in your destination country, the plan will provide for transportation to the nearest place of safety or for return to your Home Country. You must contact HCCMIS within 10 days of the date the travel warning is issued and the evacuation must be approved in advance and coordinated by HCCMIS.

    Emergency Medical Evacuation:

    If recommended by your attending Physician, who certifies that Evacuation is necessary to safeguard your life and that Medically Necessary treatment is not available locally, and if approved in advance and coordinated by HCCMIS, the Atlas Series will provide the following benefits: Emergency air and/or ground transportation to the nearest Hospital that is qualified to provide the Medically Necessary treatment.

    Emergency Message Relay
    We will relay messages to your family, friends and co-workers, helping you to maintain contact during an emergency.
    Emergency Reunion:In the event of a covered Emergency Medical Evacuation, the Atlas Series will provide the following benefits: The cost of an economy round-trip air and/or ground transportation ticket for one of your Relatives (parent, spouse, sibling or child age 18 or older) for travel to the area where you are hospitalized following Emergency Medical Evacuation and reasonable expenses for lodging and meals for your relative, for a period not to exceed 15 days.

    Return of Minor Children:

    If you are the only person age 18 or older, traveling with one or more children under the age of 18, who are also covered by the Atlas Series, and you are Hospitalized for treatment of a covered Illness or Injury resulting in the children being left unattended for a period of time expected to exceed 36 hours, the Atlas Series will provide the following benefit: The cost of a one-way economy air and/or ground transportation ticket for each covered child to the terminal serving the area of Principal Residence of each covered child.

    Terrorism

    The Atlas Series provides Medical coverage for Injuries and Illnesses resulting from an Act of Terrorism, subject to a $50,000 lifetime maximum, provided all of the following conditions are met:

    1. The Injury or Illness does not result from chemical, nuclear or biological weapons or events.
    2. You have no direct or indirect involvement in the Act of Terrorism.
    3. The Act of Terrorism is not in a country or location where the United States government has issued a travel warning that has been in effect within the 6 months prior to your date of arrival.
    4. You have not unreasonably failed or refused to depart a country or location following the date a warning to leave that country or location is issued by the United States government.

    An Act of Terrorism is defined as: an act, including but not limited to, the use of force or violence and/or the threat thereof, of any person or group(s) of persons, whether acting alone or on behalf of or in connection with any organization(s) or government(s) committed for political, religious, ideological or similar purposes including the intention to influence any government and/or to put the public, or any section of the public, in fear.

    Accidental Death and Dismemberment:
    In the event of your Accidental Death (except while traveling on a common carrier) or Dismemberment resulting from a covered Injury, the Atlas Series will provide the following benefit:

    • Accidental Death – Principal Sum to the Beneficiary designated on your Application
    • Loss of 2 eyes or 2 or more limbs – Principal Sum to you
    • Loss of 1 eye or 1 limb – One-half of the Principal Sum to you

    The Accidental Death and Dismemberment benefit is not available for losses incurred during participation in a Hazardous Sport or in respect to losses resulting from an Act of Terrorism.

    Common Carrier Accidental Death:

    In the event of your Accidental Death while traveling on board a commercial common carrier, the Atlas Series will provide the following benefit: Principal Sum of $50,000 ($25,000 for children under age 18), subject to a maximum of $250,000 per family, to the Beneficiary designated on your Application. This benefit is not available in respect to losses resulting from an Act of Terrorism.

    Repatriation of Remains:

    In the event of a covered Injury or Illness resulting in your death, the Atlas Series will provide the following benefit: Air and/or ground transportation of bodily remains or ashes to the area of your Principal Residence, and reasonable costs of preparation of your remains necessary for transportation.

    Natural Disaster:

    In the event of natural disaster (hurricane, flood, tornado, tsunami, etc) the Atlas Series will provide you up to $100 a day for 5 days if you are Displaced from planned, paid accommodations due to evacuation from forecasted disaster or following a disaster strike. Displaced is defined as required to depart the destination due to an evacuation ordered by prevailing authorities. Proof of paid accommodations must be submitted at time of claim.

    Lost Passport or Travel Documents Assistance
    Losing important documents is extremely troublesome. We can help you notify the appropriate authorities for assistance with replacement of documents.
    Trip Interruption:

    1. If, after you have departed, you learn of the death of a parent, spouse, sibling or child, or you learn of the substantial destruction of your Principal Residence by fire or weather, the Atlas Series will provide the following benefit: The cost of an economy, one way air and/or ground transportation ticket for your travel to the area of your Principal Residence; or
    2. If, following a covered Emergency Medical Evacuation, the attending Physician states that it is Medically Necessary for your return to your Home Country or to the area from which you were initially evacuated for continuing treatment, recuperation and recovery, the Atlas Series will provide the following benefit: The cost of an economy, one-way air and/or ground transportation ticket for your travel from the area where you were hospitalized following the Emergency Medical Evacuation to the area where you were initially evacuated from, or to the terminal serving the area of your Principal Residence.

    We will make emergency travel and itinerary changes for you including rebooking flights, hotel reservations and ground transportation arrangements.
    Lost Checked Luggage:

    In the event your checked luggage is permanently lost by the transportation provider, the Atlas Series will provide the following benefit: Up to $250 for replacement of clothes and personal hygiene items, not to exceed $50 for any one item. You must file a formal claim with the transportation provider and submit copies of all claim forms and proof that the transportation provider has paid you its normal reimbursement for the lost checked luggage.

    What Is Excluded?
    The following charges, treatments, surgeries, medications, conditions and circumstances are excluded:

    1. Pre-existing Conditions – Charges resulting directly or indirectly from any Pre-existing Condition are excluded from this insurance. If you are under age 70, you are covered for Medical and Emergency Evacuation charges resulting from an Acute Onset of a Pre-existing Condition, up to the limit set forth in the Schedule of Benefits and Limits. A Pre-existing Condition is any Illness, Injury or medical condition or chronic or recurring Illness or Injury or medical condition, including any associated complications or consequences, which existed at or during the 2 years immediately preceding your Effective Date. An Acute Onset is a sudden and unexpected outbreak or recurrence of a Pre-existing Condition, that occurs spontaneously and without advance warning either in the form of Physician recommendations or symptoms. Treatment must be obtained within 24 hours of the sudden and unexpected outbreak or recurrence
    2. Treatment for or related to any congenital condition
    3. Routine pre-natal care, childbirth, care of newborns, post-natal care, birth control, artificial insemination, infertility, impotency or sexual dysfunction, sterilization or reversal thereof
    4. False labor, edema, prolonged labor, prescribed rest during the period of Pregnancy, morning sickness and conditions of comparable severity associated with management of a difficult Pregnancy, and not constituting a medically distinct Complication of Pregnancy, and all charges related to Pregnancy after the 26th week of Pregnancy
    5. Mental Health Disorders or Substance Abuse
    6. Charges which are not Incurred during the Certificate Period or the applicable Benefit Period, and charges which are not presented to Underwriters for payment within 60 days from the end of the Certificate Period or the applicable Benefit Period
    7. Charges for use of Emergency Room within the US for treatment of Illness unless the patient is directly admitted to the Hospital as Inpatient for further treatment of that Illness
    8. Not Medically Necessary and administered or ordered by a Physician
    9. Provided at no cost, by a family member, or by a person who ordinarily resides with you, or which are attributable to or recoverable from any other party including government-sponsored plans
    10. Charges which exceed Usual, Reasonable and Customary
    11. Investigational, Experimental or for Research Purposes
    12. While confined primarily to receive Custodial Care, Educational or Rehabilitative care
    13. Venereal Disease, and treatment of individuals who are HIV+ or have AIDS or ARC
    14. Treatment by a Chiropractor
    15. Diseases of the skin
    16. Dental treatment, including treatment of the temporomandibular joint, except for Emergency Dental Treatment necessary to replace sound natural teeth lost or damaged in an Accident covered hereunder or for the relief of acute, spontaneous and unexpected onset of pain
    17. Eyeglasses, vision exams, contact lenses, hearing tests, hearing aids, hearing implants, eye refraction, visual therapy, orthoptics or visual eye training or eye surgery (including cataract surgery and radial keratotomy) or for any examination or fitting related to these devices or procedures
    18. Injury resulting from participation in the following activities: Amateur Athletics, Contact Sports, and professional sports or athletic activities. Non-contact and non-organized/non-sanctioned amateur sports or athletic activities engaged in by the Member solely for leisure, recreational, entertainment or fitness purposes are not excluded except the following which are excluded: Mountaineering where ropes or guides are normally used or at elevations of 4,500 meters or higher. Aviation, except when traveling solely as a passenger in a commercial aircraft. Hang gliding, sky diving, parachuting or bungee jumping; Snow skiing or snowboarding, except for recreational downhill and/or cross-country snow skiing or snowboarding (no cover provided whilst skiing away from prepared and marked in-bound territories and/or against the advice of the local ski school or local authoritative body); Racing by any animal or motorized vehicle; and spelunking; and subaqua pursuits involving underwater breathing apparatus unless NAUI/PADI certified, accompanied by a certified instructor, and at depths of less than 10 meters; jet skiing; and any other sport or athletic activity which is undertaken for thrill seeking and exposes you to abnormal or extreme risk of injury
    19. Injury sustained while under the influence of or due wholly or partly to the effects of intoxicating liquor or drugs other than drugs taken in accordance with treatment prescribed and directed by a Physician but not for the treatment of Substance Abuse
    20. Willfully self-inflicted Injury or Illness and immunizations and Routine Physical Exams
    21. The Deductible, Coinsurance and charges which are not included as Eligible Expenses as described in the Master Policy, and charges which exceed the limits set forth in the Schedule of Benefits and Limits
    22. Treatment required as a result of complications or consequences of a treatment or condition not covered hereunder
    23. Charges for travel or accommodations, except as provided for in the Local Ambulance, Emergency Medical or Political Evacuation, Repatriation of Remains, Emergency Reunion, Natural Disaster and Trip Interruption sections of this insurance
    24. Treatment incurred as a result of exposure to non-medical nuclear radiation and/or radioactive material(s)
    25. Organ or tissue transplants or related services
    26. Acts of Terrorism, except as provided for herein, war, insurrection, riot or any variation thereof

    What if I Plan to Participate in a Sport or Athletic Activity that is Excluded?
    The Optional Hazardous Sports Rider is available for the adventurous traveler for an additional 20% premium.

    This Optional Hazardous Sports Rider adds coverage for the Amateur sports listed in exclusion #18. The maximum coverage under this Rider is the Overall Maximum Limit you select. The Accidental Death and Dismemberment benefit is deleted during the course of the activity.

    What are the Pre-certification Requirements?
    Hospitalizations, Surgeries, Emergency and Political Evacuations, Emergency Reunions, Trip Interruptions, Repatriation of Remains, Computerized Tomography (CAT Scan) and Magnetic Resonance Imaging (MRI) must be Pre-certified. Simply call, or have your Physician call, HCCMIS with the information relative to your claim. You may also pre-certify by submitting details through Client Zone. Be sure to have your ID number available. If you do not Pre-certify, medical expenses will be reduced by 50% and all other expenses will be forfeited.
    How is Pre-certification handled in the case of an emergency or natural disaster?
    You, a family member, your Physician or anyone else has up to 48 hours after the event, or as soon as possible, if there’s no ability to contact HCCMIS (ie: natural disaster) about what happened.

    How do I Extend or Renew my Coverage?
    After your initial purchase, you may extend your coverage (5 day minimum) up to a maximum of 12 months from the initial effective date. Provided there is no break in coverage, you will not be required to re-satisfy the Deductible and Coinsurance nor will benefit limits be reset.

    If you are covered under Atlas International, after 12 months of continuous coverage you may renew your coverage for up to 12 additional months.If 24 months of continuous coverage is maintained, a final period of up to 12 months may be purchased.Deductible and Coinsurance must be re-satisfied as of each renewal date.

    After 36 months of continuous coverage under Atlas International, or 12 months of continuous coverage under Atlas America, or any break in coverage, a new plan must be purchased.A new Application is required and you must re-satisfy your Deductible, Coinsurance and Pre-existing Condition provisions.

    Can Coverage Be Cancelled?
    If for any reason you wish to cancel your policy, you must submit your cancellation request in writing to HCCMIS in order to receive a refund of premium.To be eligible for a full refund, the request for cancellation must be received prior to your effective date.Cancellation requests received after the effective date will be subject to the following conditions:

    1. a $25 cancellation fee will apply; and
    2. only the unused portion of the plan cost will be refunded; and
    3. only members who have no claims are eligible for premium refund.
